SHALYATANTRA

Introduction of Department

Shalya Tantra is one of the eight foundational branches (Ashtanga) of Ayurveda, primarily dedicated to surgical and para-surgical techniques. In an Ayurvedic medical college or hospital, the Shalya Tantra department serves as the hub for surgical education, trauma management, and the treatment of diseases that cannot be cured by medicine alone.

The word Shalya refers to "foreign bodies" or "anything that causes pain/distress to the body or mind," while Tantra means "science" or "technique." Therefore, Shalya Tantra is the science of removing such harmful factors.

Department Vision

  • To establish Shalya Tantra as a globally recognized surgical discipline by blending ancient Ayurvedic wisdom with modern scientific advancements.
  • To produce highly skilled, ethical, and compassionate Ayurvedic surgeons (Shalyachikitsak) who are proficient in both classical and contemporary surgical skills.
  • To provide a "pain-free" and minimally invasive surgical experience that treats the patient as a whole—considering their physical, mental, and spiritual well-being.

Department Mission

  • Clinical Competency: To provide rigorous, hands-on training to undergraduate (BAMS) and postgraduate (MS-Shalya) students in both classical Ayurvedic surgical procedures and modern general surgery.
  • Sushruta’s Legacy: To ensure students master the use of traditional instruments (Yantras and Shastras) and the 60 methods of wound management (Shashti Upakrama).
  • Ethical Practice: To instill a strong sense of medical ethics, surgical discipline, and professional responsibility in future surgeons.
  • Minimally Invasive Care: To prioritize "Para-surgical" techniques like Ksharasutra, Agnikarma, and Jalaukavacharana (leech therapy) to provide cost-effective treatments with minimal side effects and low recurrence rates.
  • Standardization: To implement Standard Operating Procedures (SOPs) for all surgeries to ensure patient safety and high-quality outcomes.
  • Holistic Recovery: To focus not just on the surgery itself, but on pre-operative (Purva Karma) and post-operative (Paschat Karma) care using Ayurvedic medicines to speed up recovery and tissue regeneration.

Future Plan

  • AI-Driven Diagnostics: Implementing Artificial Intelligence and machine learning to analyze imaging data (CT/MRI) for more precise preoperative planning in complex fistula and tumor cases.
  • 3D and 4D Imaging: Utilizing advanced radiological tools to visualize anatomical structures in real-time during traditional Shalya procedures.
  • Virtual Reality (VR) Training: Developing VR simulations for students to practice Sushruta’s surgical techniques (like the Ashta Vidha Shastra Karma or eight types of surgical acts) in a risk-free digital environment.
  • Laser-Assisted Kshara Karma: Developing "hybrid" treatments that combine modern laser technology with classical Kshara Karma (alkaline therapy) for faster recovery in anorectal disorders.
  • Electric Agnikarma Units: Replacing traditional metal rods (Shalakas) with precision-controlled electrical and infrared thermal units to ensure consistent temperature and hygiene.
  • Smart Ksharasutra: Researching new medicated coatings for threads that utilize nanotechnology to release healing herbs more effectively into the fistula tract.
  • Herbal Hydrogels: Developing advanced wound dressings infused with Ayurvedic herbs like Jatyadi or Katupila to treat chronic diabetic ulcers and non-healing wounds.
  • Standardization of "Shashti Upakrama": Creating globally accepted scientific protocols for the 60 classical methods of wound care to make them a standard in modern dermatology and surgery.
  • Independent Practice of 58 Procedures: In line with recent Indian government (NCISM) notifications, the department's future plan involves training PG scholars to independently perform nearly 39 surgical and 19 additional procedures, ranging from general surgery to ophthalmology.
  • Laparoscopic & Endoscopic Integration: Incorporating minimally invasive modern surgical tools into the Ayurvedic OT to offer "Integrative Surgery" for gallstones, appendicitis, and hernia.
  • Evidence-Based Protocols: Conducting large-scale clinical trials to publish data in international journals, aiming for WHO-level recognition of procedures like Jalaukavacharana (Leech Therapy) for vascular disorders.
  • Surgical Skill Labs (Yogya): Establishing dedicated labs where students use mannequins and synthetic models to master suturing and incision techniques before touching a patient.
